**This is a borrower authorisation form which enables the borrower's Intermediary to be given authority by the borrower to obtain necessary information relating to the mortgage on a renewable 12 month basis.

In order your applications can be processed quickly, please can you assist by answering all sections of the application form, including the budget planner, and ensure all are fully completed. If any items of expenditure are not incurred please put N/A.

If any data is missing, the application form will be returned to you, which could delay the processing of the application.
